Care@Home.
A nurse practitioner or physician assistant comes to your home to give you extra attention, coordinated with your Breakwater Primary Care provider. Home based physical therapy, nursing, and social services available so you can get a high level of care in your own home.
Advanced Illness Program.
All the benefits of the Care@Home Program with even more intense attention from specially trained nurses who excel at caring for people in stages of advanced illness and palliative care so that you and your family can receive the highest quality of care and support in the home.
Heart Health Program
Partnering with the Cardiovascular Institute of New England, this program helps to empower patients to change heart failure to heart health, by quickly getting patients on the right medicines in therapeutic doses, provide health coaching including dietary sodium and volume management, same day appointments with a cardiologist for urgent needs, and timely access to important treatments like IV Lasix.
Remote Patient Monitoring
Have your blood pressure, weight, and oxygen levels monitored at home, with readings sent to our nurse care manager, who will track progress and notify your Breakwater Primary Care provider with any concerns.
Cancer Support
Partnering with the Daymark Health, this program helps to support patients and their families diagnosed with cancer to navigate the complex world of testing and treatment options, while providing emotional support through the process.
